I first obtained a copy of the third edition of this book in , just prior to publication of the fourth edition, and it has been a regular companion since, helping both in my therapeutic training and in the subsequent development of teaching material for undergraduates and postgraduates. You may appreciate why I jumped at the opportunity to review the latest edition and understand why, although it remains an outstanding book in this area, I was a little disappointed in this latest revision, despite the fact that its structure has changed little over these three editions. While these names have become synonymous with the book, as with previous editions the primary editors have assembled a formidable combination of optometrists, pharmacists, medical practitioners and researchers to help compile and revise it. The book is divided into four sections:

Each chapter is well supported by references with an emphasis on sources from peer-reviewed literature. The attractive style with bolder print has meant some compromises where text shown in tables has been abbreviated and where for this latest edition comparative figures have not been reproduced on adjacent pages.

Although the use of chemical structures may not be considered clinically relevant I would like to have seen a few more retained.

Structures of antivirals in the previous edition, for example, usefully highlight structural similarities to target DNA nucleosides. Some data, such as the chart of commonly used abbreviations is only relevant to American practice and indeed use of these in the UK could lead to dispensing errors.

Another minor criticism is the occasional inconsistency with trade and generic drug nomenclature.

I am also frustrated that it has become difficult to find the original source of particular pieces of information from the book. I am forced to acknowledge that these revisions probably reflect a trend to producing shorter references that are more practical for use in the consulting room. There are suggestions to this effect in the authors' preface.

I am pleased to report that most, if not all, of the updates properly reflect recent clinical advances and altered drug availability since the fourth edition was published. This new chapter is devoid of figures, images or tables to illustrate the scientific basis for the use and success of these drugs. There is not the level of reflection on the therapeutic implications of the major treatment trials of glaucoma or ocular hypertension that might be expected given this book's pedigree and there is an inexplicable removal of management segments covering pseudoexfoliative and pigmentary glaucomas.

When I initially flicked through the book, I was immediately disappointed with the quality of production. In this edition there is an emphasis on the need to report any suspected adverse events although the mechanism for reporting is only relevant to the USA.

The final chapter on the management of systemic emergencies has been expanded to include drug-related reactions and offers defensive strategies to prevent or minimise adverse drug events.
